The Jubilee Park Community Clinic offers comprehensive community health care services, including medical care that covers everything from routine check-ups and vaccinations to chronic condition management.

Located on the first floor of our spacious, two-story clinic, Parkland Health provides primary care for all ages. Whether you need preventive care or ongoing medical attention, our clinic is here to support the health and well-being of our community.

The second floor houses mental health counseling through Jewish Family Service, health mentoring services, and a variety of health education programs designed to promote overall wellness.

How Community Health Care Services Work

Clinical services provided by Parkland Health and Jewish Family Service at the Jubilee Park Community Clinic have their own intake and insurance requirements. For questions or more information, including any available assistance programs, please contact the provider directly. Please call (214) 226-4000 for Parkland Health and (214) 887-1364 ext. 272 for Jewish Family Service. All programming provided through Jubilee Park & Community Center’s Health & Wellness Team is offered at no cost to all registered participants. This includes yoga, Zumba, youth sports, cooking classes, parenting support classes, and more!

Appointments

Appointments are required for all medical, dental, and counselling services.
For medical care, visit the clinic (820 Ann Ave.) in person to make an appointment or call Parkland Health directly.

For dental care, patients must first be seen by a doctor at the clinic (820 Ann Ave.), and only then can they be referred for dental services.

Medical services are restricted to residents in ZIP codes 75223, 75210, and 75227.
Dental care is restricted to residents in ZIP codes 75223 and 75210.
Mental health services are restricted to residents in ZIP codes 75223, 75210, and 75227.

What to Bring

No specific paperwork is required by Jubilee, though Parkland Health and Jewish Family Service have their own documentation processes, which will be addressed when appointments are made.
